    Job Overview

    The Clinical Pharmacist at Dr SK Matseke Memorial Private Hospital will be responsible for leading, coordinating, and improving clinical pharmacy services. This role involves daily patient interactions through ward rounds and medication reviews, which are essential to optimizing patient outcomes. The ideal candidate will leverage their knowledge and experience in clinical pharmacy, particularly in areas like antimicrobial stewardship and pharmacovigilance, to enhance pharmaceutical practices and improve patient care.

    Key Responsibilities

    The Clinical Pharmacist will:

    1. Provide Patient-Centric Clinical Pharmacy Services: Conduct daily ward rounds to perform comprehensive medication reviews. This involves monitoring patient responses to medications, identifying potential issues, and making timely interventions to ensure safe and effective therapeutic outcomes.
    2. Drive Antimicrobial Stewardship: Antimicrobial stewardship is critical in managing drug resistance. The Clinical Pharmacist will monitor antimicrobial usage, collect relevant data, and make evidence-based recommendations to prevent inappropriate antimicrobial utilization. This role also includes collaboration with infection prevention teams to optimize medication use and minimize risks.
    3. Optimize Drug Utilization: The Clinical Pharmacist will implement interventions that enhance pharmaceutical care, ensuring that medications are used judiciously and effectively to improve patient outcomes.
    4. Collaboration with Multidisciplinary Teams: The role requires active collaboration with various stakeholders, including doctors, microbiologists, nursing staff, and infection control personnel, to foster a cooperative approach to healthcare. This teamwork ensures the continuity of care and the alignment of treatment goals.
    5. Provide Expert Drug Information: The Clinical Pharmacist will provide evidence-based drug information, supporting safe and effective medication therapy management. Participate in Internal Stakeholder Meetings: Regularly attending and contributing to internal meetings, presenting updates on progress and identifying areas for improvement, is an essential component of this role.     Qualifications and Experience

    Candidates for the Clinical Pharmacist position must meet the following requirements:

    • Educational Background: A Bachelor of Pharmacy (B. Pharm) degree along with a postgraduate qualification in Clinical Pharmacy.
    • Registration: Active registration with the South African Pharmacy Council is mandatory.
    • Experience: Five years of experience as a pharmacist (post-internship) is required, with at least two years in Clinical Pharmacy, focusing on areas such as Antibiotic Stewardship, Pharmacovigilance, and Rational Medicine Use.
    • Technical Skills: Proficiency in Microsoft 365 applications, including Excel and PowerPoint, is essential. Familiarity with the Blue Bird programme and SAP software will be beneficial.
    • Interpersonal Skills: Excellent communication skills, both written and verbal, are required to effectively collaborate with healthcare teams and deliver patient-focused services.
